How can I make each entry be associated to a team and provide the affiliation?

In the final submission, each team is required to submit their results and write up a factsheet providing details of the team and affiliation. In this phase, it is not allowed for one team to submit serval entries using different Codalab accounts. Otherwise, the submission will be disqualified.
